Chapter Annual Report  Due May 1, 2021

This report should be completed by the Chapter President or their designee. This reporting form is a new format for 2020 which combines an older version of the Annual Report, the Chapter of Merit Award Application, and the Achievement in Membership Award Application. 

Some data such as a membership statistics will be added to your report at the National level. We encourage every chapter to share at least one success story in this report. There is an option to share multiple stories if you desire.
